First prosecution for breach leading to a fatality under the WA WHS Act

W.A.S.S. Nominees Pty Ltd operates with two other companies, informally known as the WA Salt Group, to mine and process salt. After mining, the salt is sent along a salt pit conveyor to processing facilities. The conveyor belt had a habit of slipping during cool and wet conditions. Plant operators would sometimes solve this problem by switching the conveyor off and on. If that did not work, the operators would place cardboard between the drive drum and the belt to stop the slipping, removing guarding in order to do so.
