Failure to install interlocked guarding on plant results in amputation injury

The Case

WorkSafe Victoria v Goulburn Valley Regional Water Corporation (2023)

Goulburn Valley Regional Water Corporation (GVRWC) operates a wastewater management facility. All inbound sewage to the workplace enters through an item of plant known as the 'inlet structure', which has a series of moving parts. The plant's moving parts are contained behind a guard that should have been fixed with bolt fastenings.

At the time of the incident, an employee performing maintenance work observed that the guard was open and solid materials had spilled into the screw press.
