16-year-old’s work experience ends in tragedy

Asphalt-laying business Pivotal Plant Pty Ltd engaged a 16-year-old volunteer worker who was undertaking work experience. The work experience included a number of tasks, such as performing labouring duties. On several occasions, the 16-year-old had operated a Sakai roller to compact newly laid asphalt.

The roller was not equipped with a well-known interlock device. This defect meant the roller could be started and commence travelling without an operator remaining in the operator’s seat.
