In or about early August 2021, A & J Morris Constructions Pty Ltd engaged Kane Atkins Plumbing Services to undertake stormwater and roofing works for a renovation and extension project. Kane Atkins Plumbing Services commenced works on or about 16 August 2021.
On 20 August 2021, a WorkSafe inspector drove past the workplace and witnessed three people standing on the roof of a single-storey building, without any visible fall protection.
